Care Management

Care Management is designed to assist patients and their support systems in managing medical conditions and related psychosocial issues by communicating effectively with the aims of improving patients' functional health status through enhancing the coordination of care with available resources in a timely and cost-effective manner.

Our case managers and social workers provide individualized services including:

  • Inpatient care management
  • Support to inpatients and family members, including crisis intervention and grief support.
  • Educating and assisting with discharge planning.

We offer a wide variety of information available to the general public and our community.
